Fees for Non Covered Services

The following NON-FACE-TO-FACE SERVICES will incur a fee.  Charges will vary according to time spent and complexity of request.

 Phone Consults initiated by patients that will be charged a fee

•  Calls to discuss test results that require more than just a few minutes of explanation or that involve a referral or require more tests.

•  Calls to review treatments plans and ask additional follow-up questions.

Prescription refills requested outside an office visit will be charged a fee

•   Refills of existing some prescriptions (does not include Dr. Adrian's Bio Identical Hormones or NP Thyroid)
•   Replacing a lost prescription.

o   No prescriptions will be refilled on Saturday, Sunday or Holidays.

o   No prescriptions will be approved if it has been 4 months since your last office visit.

o   All controlled substance prescriptions require an office visit every 6 months.

o   Existing prescriptions when there are insurance changes or pharmacy changes

     (Example: patient is on vacation and needs a prescription called in)

Lab Scripts that will incur a fee

•   Replacing a lost lab script

Test Results that will incur a fee

•   If patients request copies of their labs or test results mailed to their home.

Prior authorizations outside an office visit may incur a fee

•   Medications that require prior authorizations.

Missed appointments with no 24-hour notice will incur a fee

•   $25 for missed follow-up appointments
•   $50 for missed new patient appointments
