Fees for Additional Services

FEES will be incurred for the following Additional SERVICES

Email questions post annual or follow up appointment

• All follow up questions must be part of a medical note and require an appointment.  

Consultations initiated by patients will incur a fee ranging from $45 - $135 depending on time and complexity.  

•          Test results that require more than just a few minutes of explanation or that involve a referral or require more tests.
•          Review treatments plans and ask additional follow-up questions
•          Questions leading to a diagnosis of a new problem
•          Advice about an existing problem or recurrent problem

Forms Completion requests require an office visit:

•          School forms
•          Camp forms
•          Sports participation forms
•          Employment Physical forms
•          Disability paperwork
•          FMLA forms
•          Life Insurance forms
•          Referrals

Prescription refills requested outside an office visit will incur a fee ranging from $45 - $135 depending on time and complexity.  

•          New prescriptions or prescription changes all require an office visit and must be made part of the patient record.
•          Replacing a lost prescription.

o   No prescriptions will be refilled on Saturday, Sunday or Holidays.

o   No prescriptions will be approved if it has been 4 months since your last office visit.

o   All controlled substance prescriptions require an office visit every 6 months.

o   Existing prescriptions when there are insurance changes or pharmacy changes (Example: patient is on vacation and needs a prescription called in)

Lab Scripts will incur a $25 fee

•          Replacing a lost lab script if we did not keep a copy in your chart

Test Results review resulting in a new treatment will incur a fee ranging from $99 - $135.

•          To ensure the confidentiality of your protected health information and to give you the time and attention you need from Dr. Adrian, you will be asked to schedule a follow-up appointment to discuss

                      your test results (including lab work).

Prior authorizations outside an office visit will incur a fee of $99 - $135 depending on time required to complete

•          Medications that require prior authorizations.  Depending on your insurance this process may involve several steps.  You may be required to make an appointment to complete this process.

Email consultations:

•          No Email consultations.  All questions must be reviewed as part of an office visit and made part of the patient record. 

Missed appointments with no 24-hour notice

•          $25 for missed follow-up appointments
•          $50 for missed new patient appointments
