Office Fee Structure and Insurance Fees

Full payment is required at the time of the visit. The cost of the visit will depend upon the length of time the doctor spends with you and the complexity. As the first visit is often comprehensive, the visit may last from 45 - 60 minutes.

  • Initial consultations typically cost $290.  If more time is desired or necessary, there may be an additional charge above $290.  Many patients that see Dr. Hohenwarter have seen countless other doctors beforehand requiring extensive time to review medical records and history.  Patients are expected but not mandated to have follow up appointments to monitor their progress.
  • Follow-up appointments and telephone consults (for long distance PA patients) are billed by time, typically $99-$190.
  • A brief phone consult for questions may be scheduled and range $49 - $66.
  • EDTA Chelation Therapy is $150 per session (There are no mandatory appointments or other charges).

Methods of Payment
Accepted forms of payment include Cash, Checks and Credit Cards. Please note that Credit Card users will pay an additional 3% convenience fee.  

Cancellation Policy
If you cancel your appointment with less than 24 hour notice you will be charged a cancellation fee as follows:

$25 for follow-up visits.
$50 for new patient appointments.

This cancellation policy is strictly enforced. If you cannot make your appointment, please call and inform us so other patients have the opportunity to utilize this time.

Medicare
We are accepting Medicare patients on a fee for service basis. As of April 1, 2012, the practice no longer participates in Medicare. Patients with Medicare may see the doctor under a private contract and must agree not to submit any claims to Medicare.

Even though Dr. Hohenwarter has 'opted out' of Medicare, all lab tests or other diagnostic tests are covered as usual per your plan. Only the services and medications provided by Dr. Hohenwarter are not covered. 

Medicare Part D

Compounded Bio-Identical hormone replacement (HRT) and natural desiccated thyroid (NP Thyroid) are not FDA approved and therefore not covered by Medicare Part D.  Patients may not submit claims for reimbursement for these prescriptions.

Medicare Part D generally covers FDA approved prescription drugs with the exception of some drug categories that are excluded by law (e.g., cough/cold and weight loss drugs).  There is no list of Part D covered drugs, as each prescription drug benefit plan has its own formulary. 

Medicare Advantage plans

Medicare payment cannot be made directly or indirectly for services furnished by an opt-out physician, except for certain emergency and urgent care services.  Therefore, no payment may be made under Medicare or under a Medicare Advantage Plan for the services furnished by an opt-out physician.

All other insurance

Patients in regular indemnity insurance programs can submit their receipt on their own to be reimbursed as payment is required at the time of the visit. Many HMO's and PPO's allow partial compensation (please contact your program.) Initial consultations last up to 1.5 hours and fees vary depending on time and complexity. Chelation Therapy fees are per session. (There are no mandatory appointments or other charges.) Phone consults are billed similar to in-office appointments.

Office Policy - Restrictions
Due to the sensitivity of our patients, PLEASE DO NOT WEAR PERFUMES, COLOGNES, FRAGRANCES, HAIRSPRAY AND CLOTHING LAUNDERED IN PERFUMED DETERGENTS IN THE OFFICE. Aroma Therapy and essentials oils are OK.
