Services Requiring an Office Visit

Forms Completion require an office visit:

• School forms
• Camp forms
• Sports participation forms
• Employment Physical forms
• Disability paperwork
• FMLA forms
• Life Insurance forms
• Referrals

Questions initiated by patients require an office visit:

• Call leading to a diagnosis of a new problem
• Call regarding advice about an existing problem or recurrent problem

Email consultations require an office visit:

• No Email consultations. All questions must be reviewed as part of an office visit and made part of the patient record.

Prescriptions outside an office visit require an office visit:

• New prescriptions or prescription changes all require an office visit and must be made part of the patient record.

Test Results require an office visit:

• In order to ensure the confidentiality of your protected health information and to give you the time and attention you need from Dr. Hohenwarter, you will be asked to schedule a follow-up appointment to discuss your test results (including lab work).

Prior authorizations outside an office visit:

• Medications that require prior authorizations may require an office visit. Depending on your insurance this process may involve several steps. You may be required to make an appointment to complete this process.
