The U.S. Food and Drug Administration (FDA) recently declared that nicotinamide mononucleotide (NMN) is not a permissible dietary ingredient that can be used in dietary supplements to support NAD levels. As a result, on February 16, Amazon gave notice they will be removing all NMN-containing products from their online platform. Amazon’s cutoff will become final on March 13, 2023. Following Amazon’s lead, more and more groups will also likely remove NMN from their platforms. The resulting consequence is that many of you and your patients will be without an NAD support solution if you have previously been using NMN in your protocols and practice. If this is your situation, then I would recommend that you evaluate Thorne’s nicotinamide riboside salt, hydrogen malate, as your replacement ingredient for NAD support.

 

The Science

 

When looking at the literature regarding NMN, the original, preclinical work did look promising. Studies appeared to demonstrate support for NAD formation through the same salvage pathway as nicotinamide riboside (NR), and potentially more effectively because NR must be converted to NMN in the cell to make NAD. The literature suggested there was a potential for NMN to be directly brought into the cell. However, given the nucleotide structure, critics of this hypothesis have pointed out both stability limitations and limitations in NMN’s direct cellular absorption potential. Without either converting back to NR, or have other intermediates play a role, NMN might not be effectively absorbed by the cell.

A recent paper analyzing NMN in healthy middle-aged adults was conducted using three different doses of NMN – 300, 600, and 900 mg per day. The study showed there is a significant change in blood NAD levels over the course of 30 and 60 days of supplementation. At first blush, this result might appear to be the very evidence needed to demonstrate that NMN in human systems does raise NAD levels. However, let’s take a closer look. In the study, we do note there is a dose-dependent change in NAD levels from 300 mg to 600 mg. However, at the 900 mg dose there is no significant difference between the lower 600 mg dose, and, in fact, we see nearly identical changes in NAD levels over time for both the 600 mg and 900 mg doses, demonstrating a steep dose-saturation effect. It is also important to note that the study showed the highest significant change in NAD from baseline was roughly 40nM. On the other hand, when looking at similar blood data for changes in NAD levels with NR, we see a drastic difference in both NAD levels and in the dose-response curve. In acute dosing, as well as long-term dosing, we see a similar dose-response curve that does not saturate even at levels greater than 1,000 mg. We also observe that similar doses of NR (1,000 mg NR versus 900 mg NMN) show a far greater dose-response, with changes in NAD levels more than 40,000nM from baseline with NR versus the 40nM from baseline shown in the most recent NMN research.

So, what does this mean? Well, it would appear there is a step between the cell’s utilization of NMN within the salvage pathway, meaning NMN is not directly utilized by the cell. If it were, then we would expect to see a larger response to NMN supplementation on NAD levels at lower doses, which is actually quite the opposite from what is seen in the overall literature.

 


The Safety

 

Prior to the FDA’s recent denial of dietary ingredient status to NMN, there was a previous effort to establish NMN as a New Dietary Ingredient (NDI). The FDA’s response to NMN’s NDI petition was that there was only limited information to conclude, at that time, that NMN could demonstrate sufficient safety to be given NDI status. In contrast, in the marketplace today NR has achieved substantial safety reviews, all of which have shown that at current supplementation levels, NR is safe. The main difference between NR’s salt form options is that dosing utilizing the malate salt has been shown to be safe at higher doses than has been demonstrated by the chloride counterparts.

Overall, supplementation with nicotinamide riboside, when comparing the relevant PK/PD relationship, indicates a 1,000-times difference in NAD response than does the latest healthy human data with NMN supplementation.
